Home Based Aged Care Services

Rural Health Team provides Occupational Therapy services for the elderly, seniors and veterans. We provide in-home Occupational Therapy throughout the Upper Hunter area. Our aim is to maximise safety and independence in the home environment, whether that be the family home or an independent living or retirement unit.

Our services include:

  • Home safety assessments including falls prevention
  • Chronic disease management
  • Equipment prescription
  • Home modification assessment and advice (Minor and Major home modifications)
  • Rehabilitation to assist the individual maximise their potential.

We provide services to those funded by:

  • DVA
  • Home Care Packages (All levels)
  • Privately funded

OT for Adults

Our occupational therapists support adults (18-64 years) to build and maintain independence in everyday activities at home, work, or in the community. We offer functional assessments and therapy to help individuals achieve goals related to safety, mobility, productivity, and engagement in meaningful activities.

We can assist with:

  • OT assessments

  • Activities of daily living (e.g., showering, cooking, managing the home)
  • Equipment prescription and training

  • Home safety and home modification recommendations

  • Energy conservation and fatigue management

  • Please note – we do not do complex home modifications for NDIS clients.

Paediatric Occupational Therapy & Speech Language Pathology Services

At Rural Health Team, we provide Occupational Therapy and Speech Pathology services for children and young people aged 12 months to 18 years. Therapy is delivered in-home, at school or preschool, in the community, or even on our farm — because we know rural families need flexible, accessible options close to home.

Our local occupational therapists are experienced in supporting children’s development and participation across a range of areas, including motor skills, sensory processing, emotional regulation, and daily routines. All of our OTs work part time and are deeply connected to the local community.

We are also fortunate to be supported by speech pathologists who offer engaging, evidence-based therapy via telehealth. To enhance outcomes and access, our Allied Health Assistants (AHAs) deliver face-to-face programs locally under the guidance of both OTs and speech pathologists. These programs help reinforce therapy goals between sessions in a familiar environment.

Equine Assisted OT

Rural Health Team are proud to be providing an innovative program including horses &/or farm into therapy here in the Horse Capital of Australia, Scone. We have two OTs who are accredited to provide this support. Equine Assisted OT is non-riding. We will be working on your goals in the presence of a horse to assist the process. The goals OT’s usually work on can include fine motor and gross motor skills, problem-solving and planning, sensory processing, self-regulation and self-care skills.

As it is an OT session, it is goal focused and the client will undergo the normal OT assessment process prior to commencing equine assisted OT. Equine Assisted OT is a great way for children and adults who find it hard to engage in mainstream therapy to achieve their goals in a low pressure, non-clinical setting.

Sense Rugby

Upper Hunter Sense Rugby is a ‘straight to treatment’ service, where we collaborate with your existing occupational therapist to assess suitability and set goals for the program. Great for those on a waitlist!

This rugby-based program helps kids who struggle with team sports, focusing on building confidence, social skills, and physical development in a supportive environment.

Bike Clinics
