To make a referral for our OT or Speech Pathology services please email hello@ruralhealthteam.au or fill out the forms below.

We are not currently accepting Exercise Physiology referrals. Excitingly, our speech pathologist will soon be returning from leave! Please feel welcome to pop a referral in.

If you prefer support with your referral please phone us on 0493 467 952 or email us at hello@ruralhealthteam.au. We will work with you to book an Intake meeting where we will complete the referral paperwork over the phone with you, and then book you in for an allocation meeting with one of our team members. 

The quickest referral method is to select the Orange ‘Refer Yourself/Your Child HERE’ Button below. This will take you to our Client Portal and allow you to register as a new client. Following this you are automatically sent our forms in email, which you must complete before we can begin our onboarding process and add you to our waitlist. Please check your JUNK Emails for these forms.

If you have already registered with the portal, you can log back in to check if your forms are online to access if you are unable to find our email. Click on the “Log in to Client Portal button”
